Work comp: File medical, rehabilitation disputes
The Department of Labor and Industry (DLI) launched Work Comp Campus, an online, modernized workers' compensation claims portal, at the end of 2020. Injured workers are now able to submit Medical Request and Rehabilitation Request forms via Campus or U.S. mail.
Submit via Campus
To submit these forms to DLI via Campus it is necessary to create a Campus account or log in to an existing Campus account. After logging in, an injured worker can enter their information about a dispute, indicating whether it is a medical or rehabilitation type, to fill in the form and submit it. Creating a Campus account also enables injured workers to monitor the status of their claim and associated documentation.
Submit via U.S. mail
If an injured worker prefers not to create a Campus account, paper Medical Request and Rehabilitation Request forms can also be mailed to DLI. The forms, available on the Work comp: Forms webpage, can be downloaded, printed and sent to DLI after completion.
