The Prevailing-Wage Project Assessment Form and Classification and Code Clarification Form may be submitted when asking for assistance from the Prevailing-Wage unit and are project-specific.

Prevailing-Wage Project Assessment forms should only be submitted by contracting authorities or grant/loan recipients along with a copy of the project specifications or grant/loan application and grant/loan award letter. 

Classification and Code Clarification forms may be completed by a contracting authority, project owner, contractor, developer or grant/loan recipient. If the form is completed by a contractor, developer or grant/loan recipient, it should be routed through the project owner or contracting authority to be submitted to the Department of Labor and Industry (DLI).

Certified payroll forms must be completed by every contractor and subcontractor on a project and submitted to the contracting authority and project owner every two weeks. DLI does not collect certified payroll forms but may demand them as part of a records request.
