The employer must complete the First Report of Injury (FROI) form. The employer has 10 days from its knowledge of a lost-time claim to report it to the employer's workers' compensation insurance company.

If the employee's disability lasts for more than three calendar-days, the insurer must file the FROI form with the Department of Labor and Industry. The employer or its insurer must provide the injured employee with a copy of the FROI form; in a lost-time claim, a copy must also be sent to the employee's union, if there is one.

The employer must also give the employee the Minnesota workers' compensation system employee information sheet at the time the employee is given a copy of the FROI form.
